How should I investigate abnormal gamma glutamyl transpeptidase (GGT) in an asymptomatic patients without risk factors for or clinical features of liver disease?
GGT has limited utility as a primary liver test. Its use is recommended in particular to establish the likely origin of a raised alkaline phosphatase.
No clear consensus exists on thresholds for further investigation
In the absence of guidance we recommend:
Mild rises (< 3 ULN) justify interval retesting
Rises > 5 ULN justify consideration of ultrasound if cause is not apparent
